Event Description
High readmissions and poor outcomes for Skilled nursing facility (SNF) populations transferring to the ED are unfortunately very common. Lack of effective information exchange and coordination between SNFs and ED staff is difficult. The packet of history sent with the patient, if it arrives intact, does little to help provide quick and easy access to meaningful data to support front line staff and rapid decisions or treatment.
