SUMMARY:CSRI HDU Capability Model - Listening Session #1
DESCRIPTION:Join Civitas Networks for Health® (Civitas) and the Consortium for State and Regional Interoperability (CSRI) for a series of two listening sessions focused on the recently published Health Data Utility (HDU) Capability Model\, developed by CSRI. As a follow up to a recent webinar hosted by Civitas and CSRI that introduced the model\, these sessions offer an opportunity for interested members and stakeholders to ask questions and share perspectives and feedback on the model’s approach to help shape its ongoing refinement. The HDU Capability Model is a stakeholder-driven framework that evaluates the capability and maturity of HDUs providing health data and analytics services across the U.S. It describes what an HDU can reliably deliver at scale\, using a weighted scoring approach that moves beyond simple maturity levels\, offering a clear view of current capabilities and priorities. \nThe model is currently open for public comment now through December 12\, 2025. We invite you to review the model at thecsri.org and join the listening sessions to help inform your comments or to provide your feedback in real-time. You can also submit your feedback by emailing hdu@civitasforhealth.org.

SUMMARY:An Integrated "Turnkey" Solution Connecting Clinical and Claims Data Webinar Hosted by MedicaSoft
DESCRIPTION:HIEs play a valuable role in the healthcare ecosystem\, aggregating healthcare data and optimizing it for use in care delivery\, public health\, and analytics. DHIN has implemented a practical approach to connecting clinical and claims data and provides data services using a FHIR based solution; this “turnkey” approach can be used in any Exchange. We will discuss the approach to connecting clinical and claims information at the aggregate as well as at the individual patient level; and will outline business related use cases that are easily enabled because of the overall DHIN solution approach and functionality. Learning Objectives: 1) Understand and highlight the practical and replicable functionality associated with a “turnkey” approach to connecting clinical and claims information in the aggregate and for individual patients. 2) Identifying possible business-related opportunities using this approach. 3) Creating a positive approach for payers\, hospitals\, and ACOs to collaborate to track and close gaps in health care delivery. All objectives will be met through actual use case examples and a visionary approach to building upon what any other system has that is legacy and in use. \nPresenters: \n\nMike O’Neill\, CEO MedicaSoft/Senior Advisor IT HIMSS NYS Chapter\nRandy Farmer\, Chief Operating Officer\, Delaware Health Information Network

SUMMARY:Civitas Member-Led Webinar | PointClickCare: Help Emergency Departments (EDs) Across Your Network to Quickly Triage and Treat SNF Transfers
DESCRIPTION:Join us on December 4\, 2024 at 11:00 a.m. ET for an insightful discussion lead Civitas member PointClickCare! \nRegister Now\nEvent Description\nHigh readmissions and poor outcomes for Skilled nursing facility (SNF) populations transferring to the ED are unfortunately very common. Lack of effective information exchange and coordination between SNFs and ED staff is difficult. The packet of history sent with the patient\, if it arrives intact\, does little to help provide quick and easy access to meaningful data to support front line staff and rapid decisions or treatment. In this session you will learn how SNF patient data can: \n1. Be easily curated and delivered immediately upon patient arrival \n2. Be seamlessly integrated and accessible in the ED track board. \n3. Arrive with key insights on the recent SNF encounter. \nIn this session you will learn how Community Health Network is advancing real time care collaboration between their ED providers and post-acute network\, through real-time data on SNF transfer patients\, seamlessly integrated into their track board and delivered immediately upon patient arrival. \nRegister Now
