SUMMARY:Realizing the Promise of RHTP: Transforming Infrastructure into Sustainable Rural Care
DESCRIPTION:Rural health care does not fail for a lack of local commitment. It fails at the systemic gaps—where clinical staff are stretched thin\, outdated IT infrastructure isolates local clinics\, and patients must travel hours for basic specialty or behavioral health services. The Centers for Medicare & Medicaid Services’(CMS) landmark $50 billion Rural Health Transformation Program (RHTP) represents a once-in-a-generation opportunity to bridge these divides. However\, with states facing pressure to obligate and deploy their initial funding\, the critical question has shifted from how much funding is available to how we maximize these investments to build a permanently connected\, resilient rural health ecosystem.\n\n\n\nSuccess requires moving beyond basic check-the-box compliance. It demands strategically aligning with emerging state-level priorities\, understanding the nuances of the RHTP funding guidelines\, and building the type of enabling infrastructure that actually changes daily clinical workflows on the ground.\n\n\n\nThis session examines what it takes to turn federal RHTP dollars into lasting clinical transformation. We will share emerging trends we are seeing across different state plans and dive into preliminary\, high-level survey results detailing how early subrecipients are mobilizing their awards. Our discussion will outline actionable recommendations on how to maximize investments in enabling technology\, accompanied by a deep dive into the specific interoperability frameworks that enable true\, cross-facility interpretability. We will walk through how these shared data channels empower both rural providers and patients with the transparent access and coordinated care they have historically lacked. \n\n\n\nThe session\, hosted by Elimu and BerryDunn\, closes with a critical roadmap for measuring and maximizing programmatic impact. Under RHTP\, the ultimate goal isn’t just implementing technology or expanding telehealth for its own sake—it is proving sustainable access\, lowering unnecessary costs\, and delivering clear\, reportable value metrics back to CMS. Attendees will leave with a clear picture of the rural health landscape under this massive federal program\, a practical framework for utilizing enabling infrastructure in their own communities\, and the precise\, value-driven language needed to justify their RHTP initiatives to boards\, state agencies\, and CMS coordinators.

SUMMARY:Lessons from Accountable Health Communities Model Participants: Preparing for Future Innovation
DESCRIPTION:What does it take to make cross-sector care coordination work in the real world?  \n   \nJoin us for the next Collaboratives in Action webinar – “Lessons from Accountable Health Communities Model Participants: Preparing for Future Innovation” – on August 25 at 12 p.m. ET as we explore lessons from the CMS Accountable Health Communities (AHC) Model with Marisol Caban (Associate Director\, Community Engagement & Capacity Building)\, Audrey Hendricks (Senior Program Manager\, Data-Driven Workflows\, Clinical Redesign Initiatives)\, and Ashley Humienny (Senior Director\, Digital Systems & Solutions) of Camden Coalition\, alongside David Kendrick (Founder of MyHealth Access Network\, Chair of the Department of Medical Informatics at the University of Oklahoma’s School of Community Medicine\, and HIE and interoperability subject matter expert).  \n   \nJust as CMS has released its final evaluation of the AHC Model—highlighting positive outcomes and valuable lessons for connecting health care and social care—this conversation will examine what made the model successful and what communities can learn moving forward. From patient navigation and community referrals to data sharing and workflow design\, these organizations helped bridge health care and social care\, demonstrating how trusted community infrastructure can improve outcomes and support whole-person care.  \n   \nAs new CMMI models emerge\, we’ll discuss the findings from the AHC evaluation and explore why HIEs\, regional health improvement collaboratives\, community-based organizations\, and other Civitas members remain essential partners in advancing health improvement.  \n   \nWebinar participants will learn:  \n✅ Key findings and lessons from the AHC Model evaluation  \n✅ The role of trusted\, neutral conveners in cross-sector collaboration  \n✅ Real-world implementation lessons and workflow challenges  \n✅ How navigation programs drove impact  \n✅ Strategies for sustaining community infrastructure beyond grant funding  \n✅ What AHC can teach us about the future of health and social care transformation  \n   \nRegister today
