SUMMARY:Bridging Public and Private: How Shared Data Infrastructure Is Closing the Gap in Care Transitions\, Presented by PointClickCare
DESCRIPTION:Health care does not fail patients at a single point. It fails them at the handoffs\, the moments between an acute stay and what comes next\, when responsibility shifts and information does not follow. Closing those gaps requires something the industry has historically struggled to build: genuine collaboration across public agencies\, nonprofit health information exchanges\, and private sector data networks. \nTexas offers a working example of what that collaboration can produce. The Texas Health Services Authority operates as a public-private bridge\, connecting government priorities\, nonprofit HIE infrastructure\, and for-profit data capabilities to serve care transitions across the state. Its encounter notification program shares data from acute and post-acute facilities across the state\, giving care teams real-time visibility into patient movement. \nThis webinar examines what it actually takes to build and sustain that kind of cross-sector collaboration. We will walk through how bringing acute and post-acute data together changes what care managers can see and do\, why the public-private structure matters for trust and adoption among subscribers\, and what happens to care transition outcomes when the data stops living in silos. Research on health information exchange confirms that community HIEs operating across organizational boundaries deliver stronger benefits than enterprise systems working within a single network. \nThe session closes with what this collaboration means for patients. When acute and post-acute data move with the patient across organizations\, care managers reach high-risk patients faster\,transitions happen with more complete information\, and the gaps where patients fall through become visible for the first time. The goal was never better data infrastructure for its own sake. It was fewer preventable readmissions\, better follow-up after discharge\, and care teams that are not flying blind at the moments that matter most. Attendees will leave with a clear picture of the patientimpact that becomes possible when public and private interests align around shared data\, and a framework for making the case for that kind of collaboration in their own communities. \nVendor support for this session is provided by PointClickCare. SUMMARY:State-to-State Roundtable: Interoperability in Practice
DESCRIPTION:Social care has shifted from a secondary concern to a major clinical and financial imperative\, making the technical “how” one of our industry’s most pressing questions. While regional policy frameworks like California’s Data Exchange Framework (DxF) offer a clear roadmap\, the operational reality of standardizing non-clinical data remains a massive undertaking for Health Information Exchanges (HIEs) across the country. \nWe are bringing together a select group of data exchange experts for a virtual cross-state roundtable. This conversation will move past the theory of interoperability and into the actual practice of it. Moderated by Findhelp’s VP of National Accounts\, Andi Gillentine\, we will explore how different regulatory environments across the country are utilizing technical standards to solve the same human problems.
