SUMMARY:CSRI HDU Capability Model - Listening Session #2
DESCRIPTION:Join Civitas Networks for Health® (Civitas) and the Consortium for State and Regional Interoperability (CSRI) for a series of two listening sessions focused on the recently published Health Data Utility (HDU) Capability Model\, developed by CSRI. As a follow up to a recent webinar hosted by Civitas and CSRI that introduced the model\, these sessions offer an opportunity for interested members and stakeholders to ask questions and share perspectives and feedback on the model’s approach to help shape its ongoing refinement. The HDU Capability Model is a stakeholder-driven framework that evaluates the capability and maturity of HDUs providing health data and analytics services across the U.S. It describes what an HDU can reliably deliver at scale\, using a weighted scoring approach that moves beyond simple maturity levels\, offering a clear view of current capabilities and priorities. \nThe model is currently open for public comment now through December 12\, 2025. We invite you to review the model at thecsri.org and join the listening sessions to help inform your comments or to provide your feedback in real-time. You can also submit your feedback by emailing hdu@civitasforhealth.org.

SUMMARY:Scaling eLTSS FHIR Implementation to Advance Whole-Person Care presented by EMI Advisors
DESCRIPTION:Across the country\, states and health information exchanges (HIEs) are working to connect health and human service systems to deliver more coordinated\, person-centered care for those who rely on long-term services and supports (LTSS). This session explores how governance\, workforce capacity\, and modern IT infrastructure can enable interoperable\, digital care planning using HL7® FHIR® standards. \nPresenters will share lessons from Missouri’s statewide rollout of FHIR-based person-centered service plan exchange for individuals with intellectual and developmental disabilities (IDD). Key takeaways include aligning use cases\, funding strategies\, and governance to support whole-person care across sectors. \nBy the end of this session\, participants will understand the policy and operational drivers behind digital transformation in long-term services and supports (LTSS)\, explore strategies for building governance and workforce capacity to support FHIR-based person-centered planning\, and learn how to apply key lessons from Missouri’s IDD ecosystem to modernize LTSS initiatives in other states or regions. \nPresenters:\n• Karen Bertodatti\, EMI Advisors\n• Michael Seda\, Tiger Institute Health Alliance\n• Angie Scott\, Lewis and Clark Information Exchange
