Portland, ME & San Diego, CA – October 28, 2025 – Civitas Networks for Health today announced that 211 San Diego which encompasses Community Information Exchange (CIE) San Diego, and San Diego Health Connect (HIE) will serve as the premier conference partner for the 2025 Civitas Annual Conference alongside other California Civitas members. Convening under the theme, “Bridging Data and Doing,” the event will take place from September 28-30, 2025, in Anaheim, CA.
The partnership marks a significant collaboration aimed at fostering cross-sector solutions that drive health improvement and innovation across the nation. By leveraging the powerful relationships and collaborative work of 211 San Diego, the partnering organizations will collectively exemplify how exchanging community-level data can drive meaningful change locally, regionally, state-wide, and nationally. There is tremendous opportunity for CIEs, Health Information Exchanges (HIEs), Community Based Organizations, Quality Improvement Organizations, and health improvement and data collaboratives to increase partnerships to further connections to advance whole person care.
There is no better state to host the conference in 2025 than California, with implementation underway of CalAIM, the groundbreaking Medi-Cal transformation initiative, which seeks to create a more coordinated, person-centered, and equitable health system. With these changes, and California’s new Data Exchange Framework to facilitate the secure and appropriate exchange of health and social services information, Civitas is thrilled to host the conference in this hotbed of innovation.
