New CMMI Models: Implications for SDOH & Interoperability

Since December 2025, CMS has introduced four new models through the Center for Medicare & Medicaid Innovation (CMMI), targeting prevention, chronic care, accountable care, and serious illness. Together, these models signal a clear shift in how payment reform is being designed—moving toward longer-term, population-level accountability rather than short-cycle pilots or condition-specific approaches.
A key underlying assumption across these models: interoperability is already in place, data flows seamlessly, and infrastructure exists to support real-time, coordinated care.
In this member-only session, Karen Ostrowski, Director, Health Policy and Partnerships (Consultant), Civitas Networks for Health®, will explore what this shift means for organizations working at the intersection of health data, care delivery, and social determinants of health (SDOH). Attendees will gain a clearer understanding of how these models may impact strategy, operations, and partnerships moving forward.
