Using Real-Time Data to Drive Proactive Postpartum SUD Care and Strengthen Rural Care Continuity

Health Information Exchanges (HIEs) are evolving beyondfacilitatingdata exchange toservingasstrategicinfrastructure for improving healthcare access, quality, and outcomes. This session brings together innovative initiatives from North Carolina, Nebraska, and New Jersey thatdemonstratehow HIE data can be transformed into actionableinsights to support rural communities and high-risk populations. Speakers will explore how states are measuring the impact of HIE investments on health outcomes and costs, using exchange data to identify and address rural care access challenges through community-sensitive leakage analytics, and enabling proactive postpartum care for individuals with substance use disorders through real-time clinical event notifications. Together, these case studies illustrate how interoperable data can inform policy, strengthen care coordination, support community sustainability, and drive measurable improvements in population health.
