Civitas Announces Two New Workgroups in 2026!

Civitas is excited to announce the formation of two new workgroups launching this year! The Behavioral Health and Substance Use Disorder Workgroup is now active. Members are currently being identified to lead the Rural Health Transformation Workgroup and develop its charter. Read more about these workgroups at the link below.

Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model

Due Date: April 1, 2026

Organization: CMS Innovation Center (CMMI)

IMPORTANT NOTE: Applications must be submitted via the Participant Portal by April 1, 2026 for the first performance period beginning July 1, 2026; applications received after this date will be considered for a January 1, 2027 start.

The purpose of the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model is to improve care delivery and outcomes for individuals with chronic conditions by testing value-based payment approaches that strengthen care integration, promote health equity, and support scalable, evidence-based interventions across diverse care settings.

NIH: Addressing Health Disparities Among Immigrant Populations Through Effective Interventions

The purpose of the immigrant health initiative is to support research to design and implement effective interventions to enhance health advantages and reduce health disparities among U.S. immigrant populations (particularly agricultural workers, unaccompanied minors, first generation immigrants, and children of immigrant families) and address factors related to immigration experiences that affect health. SB&T Member-Specific Information

SB&T member benefits include:

SB&T members are invited to present on collaborations with HIEs or RHICs. Submit your proposal 6–8 weeks in advance via the SB&T Webinar Request Form. Submissions must include:

  • Title
  • Proposed date/time
  • Presenters’ names, orgs, and titles
  • Abstract
  • LinkedIn and Twitter handles

Note: Webinars must include an HIE/RHIC member and cannot be overly promotional.

Membership Dues & Policies

New members are invoiced following Board approval. Onboarding begins once payment is received. If joining after June 30, full dues are required with a half-rate applied to the following year’s renewal.

Membership is renewed annually for the calendar year (Jan 1–Dec 31). Invoices are issued by November 1, with reminders to update your organizational and financial info beforehand.

Yes. Each entity is considered a separate member and must apply individually. Additional contacts across parent/subsidiary groups must be approved on a case-by-case basis.

Members may withdraw at any time with written notice. However, no refunds will be issued for previously paid dues.

Membership may be terminated for non-payment or code of conduct violations with approval from the Civitas Board of Directors. Read the full policy.

Membership is valid for one year from payment and is non-transferable.

Yes — Civitas has drafted and distributed an Ethical AI Use Policy for members. Please review the policy, as we will be operating in accordance with it.
