PCDH ensures that patient health information follows individuals wherever they seek care, improving outcomes and supporting real-time, whole-person care coordination. 

What is the Patient Centered Data Home?

The Patient Centered Data Home® (PCDH) is a nationally recognized approach to linking patient information across state and regional lines. 

PCDH uses simple but powerful triggering alerts (based on where care occurs) to notify healthcare providers when a patient receives care outside their “home” HIE region — and facilitates real-time data sharing across trusted HIE networks.

How the Patient Centered Data Home Works

Step 1
A patient receives care outside their usual “home” region.
Step 2
The away facility’s Health Information Exchange (HIE) sends an alert to the patient’s Home HIE based on zip code matching.
Step 3
The Home HIE acknowledges the alert, confirms record availability, and notifies the patient’s usual care providers.
Step 4
Clinical information is exchanged securely between the Home and Away HIEs.
Step 5
Both care teams (away and home) receive updated information, improving both immediate and long-term care.
