To better understand how members are supporting the RHTP, Civitas surveyed its members who are working across health information exchange, data infrastructure, analytics, care coordination, quality improvement, and community collaboration. The findings reveal a network uniquely positioned to help states plan, implement, evaluate, and sustain rural health transformation efforts. The report highlights five key themes:
Health data infrastructure is foundational. Members emphasized that HIEs and other health data infrastructure organizations serve as the backbone for secure, scalable information sharing and urged states to build upon existing investments rather than create duplicative systems.
Interoperability and care coordination drive impact. Members identified data exchange, real-time notifications, closed-loop referrals, and whole-person care coordination as critical tools for improving outcomes in rural communities.
Analytics and measurement support sustainability. Respondents stressed the importance of demonstrating ROI, tracking outcomes, and using data to guide decision-making and long-term investment.
Sustainability remains the top concern. Across responses, members pointed to funding continuity, governance, workforce capacity, and long-term operational support as essential to ensuring lasting impact beyond federal funding.
Civitas has a role to convene, connect, and advocate. Members called for peer learning opportunities, implementation resources, policy advocacy, matchmaking, and shared frameworks to support success across states and communities.

UHIN is interested in the Interoperability Networks to Connect Services (LINCS) initiative which aims to create a connected health ecosystem for rural communities by enabling secure, standards-based exchange of health information across providers, public health, behavioral health, and community-based organizations. UHIN provides secure clinical data exchange, portal access, admission, discharge, and transfer (ADT) notifications, clinical notifications, Consolidated Clinical Document Architecture (CCDA) exchange, patient identity management and attribution, Direct Secure Messaging, public health reporting, and death notifications. UHIN also participates in national networks, including Patient Centered Data Home® (PCDH) and the eHealth Exchange.

RURAL HEALTH FOCUS AREA & CORE CAPABILITIES
WCHQ supports rural health transformation through technical assistance, quality improvement, data analysis, evaluation, and cross-organizational learning. WCHQ capabilities include helping organizations identify rural health needs and priority populations; interpret and apply health care quality and population health data; identify populations with the greatest opportunity for improvement; develop and implement improvement strategies; share tools and best practices; communicate findings; and connect providers, community organizations, and other stakeholders with similar priorities.
CURRENT RHTP-RELATED ACTIVITIES
Provides short-term technical assistance to help organizations implement rural health transformation activities. WCHQ also supports evaluation by aggregating data from Wisconsin health systems for metrics included in the state’s proposal.
