We appreciated the thoughtful discussion and the opportunity to explore how health information exchanges (HIEs) are evolving beyond data exchange to become critical infrastructure for improving health outcomes, strengthening rural communities, and supporting high-risk populations through actionable, real-time data.

During the session, attendees heard from several presenters across three unique presentations, including:
Rural Health HIE: Data to Outcomes Transformation Model, which explored how states can leverage expanded HIE infrastructure and investments to establish baseline measures, evaluate outcomes, and demonstrate the value of health information exchange in improving rural health care access and outcomes.
Rural Care Leakage & Community Impact Reporting, which highlighted how HIE data can be used to better understand care access challenges in rural communities, identify service gaps, and inform planning efforts that support long-term community sustainability and access to care.
Data-Driven Postpartum Support: HIE Infrastructure for High-Risk Populations, which showcased how real-time clinical event notifications can enable proactive outreach, strengthen care coordination, and improve postpartum outcomes for individuals with substance use disorders and other vulnerable populations.
