Here’s a quick recap of what was discussed:

Learn more about Civitas’ Programs.
  • Civitas 2025 Annual Conference Updates: Be sure to see below for details!
  • PCDH + Learning Health System Data Request: Dr. David Kendrick, CEO of MyHealth Access Network and Civitas Board Member, shared an important update on the Patient-Centered Data Home (PCDH) initiative and a new project from the Learning Health Systems Workgroup. The project aims to analyze how individuals’ health data is fragmented across HIEs and APCDs by ZIP code, with early findings showing many adults have data in six or more systems. The goal is to better understand population fragmentation, support nationwide health surveillance, and publish results in a peer-reviewed journal by the end of 2025. Civitas will follow up in the coming month with opportunities for members to contribute via a brief nationwide survey.
  • CQMC Opportunities: Erin O’Rourke from AHIP shared current opportunities for Civitas members to engage with the Core Quality Measures Collaborative (CQMC), including working group participation and upcoming deliverables.
  • Member Spotlight – West Health Institute: Amy Stuck and Tyler Kent presented West Health Institute’s Mosaic Platform, highlighting its role in value-based acute care and the integration of predictive analytics to support better health outcomes.
